Africa’s retail pharmacy sector is undergoing a quiet but profound transformation. Rapid urbanization, a rising middle class, longer life expectancy, and rising health awareness are driving sustained demand for accessible healthcare and prescription drugs. 

At the center of this expansion are large, well-capitalized pharmacy chains and pharmaceutical groups that are scaling footprints, digitizing services, and integrating vertically across manufacturing, distribution, and retail. 

From South Africa’s dominant pharmacy retailers, leveraging loyalty programs, private-label products, and in-store clinics, to Nigeria’s fast-growing pharmacy networks anchored in dense urban centers, these companies are reshaping how Africans access medicine.

Digital platforms, e-commerce ordering, telemedicine, and last-mile delivery are increasingly becoming competitive differentiators, while regulatory complexity, currency volatility, and price controls remain persistent challenges. 

Yet, for well-run operators with strong balance sheets and expansion capital, the opportunity is substantial. These ten companies stand out for their scale, resilience, and strategic positioning in Africa’s healthcare value chain.

1. Clicks Group

South Africa
Clicks is Africa’s largest retail pharmacy operator, with over 990 stores and about 780 pharmacies nationwide. Its R47.8 billion turnover is driven by scale, private-label health products, and one of South Africa’s most successful retail loyalty programs.

2. Dis-Chem Pharmacies
Country:
South Africa
Dis-Chem operates more than 300 pharmacy stores, generating R39.2 billion ($2.2 billion) in revenue. The group has built strong margins through bulk buying, wellness clinics, and an expanding online pharmacy platform targeting urban consumers.

3. MedPlus
Country:
Nigeria
MedPlus is Nigeria’s most recognizable retail pharmacy chain, with a strong presence in Lagos and Abuja. It serves middle- to upper-income customers, combining premium retail locations with professional pharmaceutical services and growing digital engagement.

4. SPAR Group – Pharmacy @ SPAR
Country: South Africa
SPAR is scaling its Pharmacy @ SPAR format, targeting 250 outlets by 2028. The model leverages SPAR’s grocery footprint, integrating healthcare into daily retail shopping while reducing standalone pharmacy operating costs.

5. Misr Pharmacies
Misr Pharmacies consistently ranks as Egypt’s largest pharmacy chain by revenue about $429 million, reflecting its unmatched scale and nationwide footprint. The company operates an extensive branch network that gives it dominance in high-density urban centers while maintaining strong procurement leverage with pharmaceutical suppliers. Its size positions it as a critical distribution artery in Egypt’s healthcare ecosystem, benefiting from recurring consumer demand, brand trust, and economies of scale in retail pharmaceuticals.

6. El Ezaby Pharmacy
Founded in 1975, El Ezaby Pharmacy has evolved into one of Egypt’s most influential healthcare retail brands. The chain pioneered value-added services such as home delivery, online ordering, and in-pharmacy consultations, reshaping how Egyptians access medicines.

7. Delmar & Attalla
Delmar & Attalla is a prominent pharmacy chain with a strong concentration in Cairo, where it has built a reputation for customer service and strategic location placement. The company’s growth has been driven by disciplined expansion, neighborhood-level market penetration, and personalized service. Its Cairo dominance provides pricing power and brand loyalty, making Delmar & Attalla a formidable player in Egypt’s competitive pharmacy landscape.

8. Goodlife Pharmacy

Kenya
Goodlife Pharmacy is a leading chain with numerous outlets across Kenya and Uganda, known for its significant reach, integration into the pharmaceutical value chain, and recent full acquisition by CFAO Healthcare in 2025 to expand access to medicines

9. Ecopharm
Country: Uganda
Founded by Ugandan pharmacist Idd Iwumbwe, Ecopharm has grown into one of the country’s most formidable retail pharmacy chains, combining physical outlets with online ordering and last-mile delivery. Backed by investments from XSML, the company has expanded access to medicines, healthcare, and beauty products while embedding pharmacist-led support into its model.

10. mPharma
Country: Ghana
mPharma, founded in 2013 by Gregory Rockson, Daniel Shoukimas, and James Finucane, with Rockson serving as the long-standing CEO, focused on revolutionizing African healthcare supply chains for affordable medicines. mPharma strengthened its East African footprint through the acquisition of Vine Pharmacy, once Uganda’s largest retail pharmacy chain. The deal marked a strategic effort to revive Vine’s scale and relevance by integrating mPharma’s technology-driven model, including inventory optimization and telemedicine services.
